Adult Social Care Services Quality Feedback

Introduction

This form is for you to feedback about care, support or equipment services for adults, for example care and support at home, residential or nursing care, day care and sitting services. 

We expect quality issues to be resolved locally, but that you send an SQF so that the issues and actions can be recorded as part of the Contract Monitoring and Quality Improvement Framework.  If you are unable to resolve the issues in this SQF you should escalate them to your manager or SOM.  If it is not possible to resolve locally with input from the SOM the actions will be taken up by the Operational Commissioning Team.

You can also tell us about gaps in our services. Information is very important as it tells us how useful the service is, how good the care is and where there may be problems or things that could be improved.

 Please complete the form as fully as possible. We usually share what you tell us with care providers to let them know :

a)  what they are doing well and to give their staff the feedback

b)  support them to improve where needed and

c)   to help us better plan what services are needed in the future.
